DISABILITY INSURANCES



          Disability insurance is a very important part of the Miami Marlins benefit program.  The disability plans are
        designed to provide you with a source of income in the event that you become disabled and unable to work.

        Short term disability benefits are only payable when you are unable to work because of non-work-related
        illness or injury.  All disability leaves will run concurrent with the Family & Medical Leave Act (FMLA) or state

        equivalent where applicable.

        Your Short-Term Disability (STD) Benefit:
             •  The Miami Marlins pay the full cost of the STD coverage premium.

            •  To be eligible for STD coverage, you must have worked for the Marlins at least 12 months and
                worked at least 1,250 hours during the 12-month period leading up to the injury or illness.

            •  If an application is approved by the Marlins, after the 5-day waiting period, eligible employees
                qualified for STD leave will be compensated at the rate of 66.6667% of their regular wages for
                the period of the disability or until the expiration of the leave, whichever is shorter.

                Vacation/Personal Time Off can be taken if the employee wishes to have full pay during their
                leave.

            •  The STD leave will be coordinated with all other leaves and policies.  An employee will not
                receive more than 17 weeks of paid or unpaid medical leave within a 12-month period.




       Your Long-Term Disability (LTD) Benefit:
           •  The Miami Marlins pay the full cost of the LTD coverage premium.
           •  You are eligible the first of the month after your hire date. If you are hired on the first of the
              month you are eligible that same day.
           •  The Elimination Period is the later of 120 days or the date your accumulated sick leave
              payments end, if applicable.
           •  If an application is approved by Unum, an Employee will receive 66.6667% of their pre-disability
              monthly earnings (minus any excludable earnings, such as social security payments) for the
              duration of his/her disability, as defined by Unum, or until retirement age.
           •  The maximum monthly benefit payment can be up to $15,000 per month.
           •  Rehabilitation and Return to Work Assistance Benefit:
                  o  Provides a rehabilitation and return to work assistance benefit for disabled employees
                      who are receiving LTD payments, and who are medically able to participate. Unum will
                      determine eligibility for this program.
