Nurse Advice Line
The Nurse Advice Line is a free service for Health Net members that connects you to a registered nurse who
can answer your health questions right away. Call our Nurse Advice line at (800) 893-5597 to help determine
when to go to an ER, Walk-in Clinic, or schedule a doctor visit for a given issue. This program offers a
convenient alternative when it’s not an emergency. You can call, or chat online with a nurse by logging on to
www.healthnet.com. (Refer to IRS Publication 969 for complete rules.)
Health Net Wellness Rewards Program
It’s easy to get rewarded! Just take the online Health Risk Questionnaire (HRQ) and share the results with your

primary care physician (PCP).
• First, log in to www.healthnet.com and complete the HRQ.
• Then, schedule an appointment for your preventive care physical with your PCP (bring a copy of your HRQ
to your appointment).
• Once you have discussed your HRQ results with your PCP, you must log back into your online account and
complete the Primary Care Physician Form to confirm you shared your results with your PCP. The PCP form
is located in the Rewards section under Complete Forms & Survey.
• Upon completion of these steps, you will be eligible for a $50 gift card reward.
