Please note that recently, Ameritas (Ameritas Insurance Company Vision) purchased the Security Life (ACI current
Vision) and the policy with Security Life will be until 11/30/2016 and Ameritas company will service the new policy
beginning 12/1/2016.

Please see below information on Ameritas.
1. Each of the members will receive the new ID cards from Ameritas.
2. The vision network will remain the same.
a. Both of Ameritas and Security Life use the same Eyemed network.
3. Each of members do not need to re enroll.
4. The plan will be similar between new policy from Ameritas vs. current policy from Security Life.
a. Further detail information to be followed by the Ameritas.
