Dear all,
The Annual Health Insurance Open Enrollment has just begun!

This is the Open Enrollment Period to add Yourself or Family Members for this year.
You can also make needed changes such as adding spouse and/or children by completing and returning the
attached Medical, Dental & Vision Enrollment/Change form to HR no later than July 16th, 2018.

You can enroll by completing and returning the attached Medical, Dental & Vision Enrollment/Change form to
HR no later than July 16th, 2018.

New enrollments and changes will take effect August 1, 2018.
Please note that if you do not enroll this time, you may not be able to enroll until July 2019 unless you have
qualifying event.
It is required to submit the signed Waiver form (Medical, Dental & Vision Enrollment Form-Page 3: E. Waiver
of Coverage) if you decline the company insurance benefit.

If you don’t make any change this time, you may default into the same or comparable coverage that you
elected last year.

 Please find attached: “Employee Benefit Handbook 2018”
The benefit handbook includes Benefit eligibility, benefit summaries, rate, and how to find a doctor in your area.
The contact information for Insurance agency, Chun-Ha Insurance and each carrier’s member service contacts are
attached for you to contact whenever you need assistance. It also contains how to log in to member portal.
The purpose of the Employee Benefit Handbook is to provide information and guidance to all employees to find a
right plan and understand your plan.

Care Dental effective
August 1, 2018:
* ACI continues with same insurance companies for Health and Vision.
* Current Dental subscribers and New Dental applicants need to FILL OUT the United Health Care Dental
application.
