        [ ACI JOB SITE SAFETY ORIENTATION ]
        To help strengthen the job site safety orientation process, ACI is updating the “ACI Safety Orientation” acknowledgment form
        .
        The updated Job Site Orientation Form will replace the current job site safety orientation forms: “New Employee Safety Orie
        ntation & Code of Safe Practices receipt”, which must be used starting 7/26/2018
        All ACI employees, ACI Subcontractor’s employees and Visitors will need to go through the job site safety orientation and s
        ign the acknowledgement form PRIOR to working and/or accessing the ACI project.


        [ACI SAFETY FIRST STICKER (ASFS) ]
        ACI will continue to maintain the ACI Safety First Sticker (ASFS) requirement at Jobsites (Hard Hat).
        Anyone who is working in your jobsite MUST have the mandatory “ASFS” on their Hard Hat after completing & signing off t
        he Job Site Safety Orientation.

        Workers need to exchange the signed “ACI Job Site Safety Orientation acknowledgement form” with the “ASFS”
        Please ensure to keep the record of names who received the “ASFS”

        ALL ACI Jobsites are required to have approx. 100 of the “ASFS”
