Please be advised that a Warning Letter for Safety Non-Compliance has been made for all project teams’ use. This letter is to
be used conjunctively with the previously issued Employee Safety Warning Report and Safety Warning Log.

 Warning Letter for Safety Non-Compliance [SUB]: This form has been made for your use to issue a warning to
Subcontractors that safety violations were made by their employee(s) on the job site. The attached form is a fillable
PDF to send warnings electronically. This form is to be sent to the Subcontractors of the violating employee.

 Employee Safety Warning Report [WORKER]: This form has been made for your use to write up all safety
violations made by Subcontractors' Employees on the job site. The attached form is a fillable PDF to send warnings
electronically. This form is to be given to the Violating Employees directly and should be attached to the
Warning Letter sent to their employer.

 Safety Warning Log: This log has been made for your use to track all safety violations made by employees and
workers on the job site. The attached log is a fillable PDF to record all events electronically.
