This is a reminder to those who have not submitted yet, you can still provide your information using the
following Cal/OSHA-approved options:
Vaccine Card
Image of Vaccine Card
Health care document showing your vaccination status
If I do not hear from you by Wednesday, June 30 , per the CAL/OSHA mandate you will be deemed
