Anzea Textiles is committed to a comprehensive benefit program that helps
our employees stay healthy and feel secure while maintaining a work/life
balance.

Anzea is pleased to offer medical, dental, vision, life, and short-term disability
coverages to our employees. Anzea covers a 75% of Employee Only medical,
dental, and vision and 100% of your basic life/AD&D and short-term disability.
Additionally, Anzea pays 25% of your dependent’s cost to make these more
affordable for our employees. Employees have the option to elect from 2

medical plans (The current PPO plan and a new HMO plan which offers a
matching benefits 27% lower rates), dental, vision, and voluntary life for family
members.

Please retain a copy of this guide in order to review coverage benefits, cost
information, and retain necessary contact information. This booklet will not

serve as an ID card or verification of benefits at a provider’s office, but can
provide assistance, if needed. For more information and to review the
Glossary of Health Coverage and Medical Terms, you can log into Anzea’s
Employee Benefit Website 24/7 with your personal ID and password

(see instructions on page 18).
