         Revelations Health Care Group is committed to a comprehensive benefit program that helps our
         employees stay healthy and feel secure while maintaining a quality work/life balance.

         Revelations  Health  Care  Group  is  pleased  to  offer  our  full-time  Group  1  employees  Three  United
         Healthcare Major Medical Options and two Pan American Limited medical lower cost options with
         Healthcare2U  (DCP)  Direct  Primary  Care  benefit  that  offers  Unlimited  $10  PCP  Doctor  Visits  and
         Unlimited  $25  Urgent  Care  visits,  see  pages  4  -  19  for  more  information.  Both  UHC  and  Pan
         American  have  Unlimited  Preventative  Care  coverage  with  NO  Copay,  Deductibles,  or  Out  of
         Pocket Costs. Our benefit package also includes a new Voluntary Dental, and Vision with Mutual of
         Omaha.    NEW  this  year  is  Voluntary  Life  with  matching  Accidental  Death  &  Dismemberment.
         Employees can apply for $150,000 with NO Health Questions, Spouses $30,000 and Children $10,000.
         Also new this year  is Critical Illness with NO Health Questions meaning you cannot be turned down.
         Finally,  we  are  offering  with  Mutual  of  Omaha    On  and  Off  the  Job  Accident  Coverage  for
         employees and dependents.


         Revelations Health Care Group contributes a portion of the monthly employee only premium for all
         of the Medical plan options.


         Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
         necessary contact information. This booklet will not serve as an ID card or verification of benefits at
         a provider’s office, but can provide assistance if needed. Refer to page 29 for instructions on how
