                                              Accident insurance helps a benefits package

                                              Today’s medical plans, with their rising deductibles and co-pays, can leave workers
                                              exposed to many of the costs of an accidental injury. Accident insurance helps a
                                              benefits package go the distance, by providing employees money they can use to
                                              help cover out-of-pocket costs resulting from a covered accident.






                                            „   You can customize the plans you offer your employees:
                                              – On/off-job or off-job only plan
                                              – Three levels of base plan benefits: Basic, Preferred and Premier
                                            „   You may also choose from four optional base benefits:
          Flexible plan designs               – Active Lifestyles, Gunshot Wound and two Wellbeing Assistance benefits

                                            „   And you can allow employees to choose from five optional riders:
                                              –  Accident Disability, Accident/Sickness Disability, Critical Illness,
                                               Sickness Hospital Confinement and Sickness Hospital Confinement with
                                               Sickness Hospital Admission


                                            In addition to important base plan benefits such as accident emergency treatment,

                                            Examples include telemedicine, post-traumatic stress disorder treatment, medical
                                            equipment and therapy.


                                            „   Guaranteed issue — for businesses with three or more eligible employees and at least one
                                              enrolled employee. Only the Accident/Sickness Disability rider includes health questions.

                                            „   Base plan is guaranteed renewable for life as long as premiums are paid when due.
                                              Disability riders are guaranteed renewable to age 75.
