Where as in the policy copy K Pad-
mavathi is noted as daughter. She is not
legally blessed with deceased. She is the
daughter of deceased wife‟s sister.
During the course of investigation, I
went to neighbours house and enquired
about the reliabilities in the above said
death claim of Mr R Ramaiah. Later I
interacted with Dr K Venkateswara Rao
who treated the deceased and issued
certificate confirming that the deceased
took treatment on 04.05.2009 and the
same was notarised. And in the next
step, I went to claimant‟s house and
inquired about the occurrence and col-
lected the necessary documents from
them. I found one lab test report which
was taken on 04.05.2009. I am herewith
submitting all the documents for your
Investigator‟s comments further determination.

(ii) Death certificate of deceased
(iii)Lab investigation report taken
on 04.05.2009
(iv)Doctors certificate with nota-
rised
(v)Copy of pass book of deceased
(vi) Voter ID Card of claimant
(vii)Addressing letter from claimant
(viii) House hold card of the
neighbours and their statements
(ix) Policy copy along with proposal
form.
During the course of enquiry made
For his/ her last illness (mention the with dissimilar individuals, the de-
period of hospitalisation, name/s of the
doctors attended and IP/OP no.) ceased was taken operation for his leg
one month prior to the date of death.
During the course of investigation,
