Application PHASES

PHASE 1: HOMEWORK

Before filing your Medicaid application these hom ewor k steps must be com plete. By having
these tasks completed prior to filing you increase your chances of having you r case expedited.

1. Complete a strategy session with on e of ou r elder law attor ney's and you r Medicaid Specialist.

2. Determine which strategy selection(s) you wish to use.
3. Execute any necessary legal documents for you r case.
4. Complete all requirements for asset restructuring.
5. Submit all requested supporting documents to you r Medicaid Specialist.

PHASE 2: APPLICATION

It's GO TIME!

Now that your homework is complete we'll
PHASE 3: RECONSIDERATION file your Medicaid application! Remember,
DCF has 30 days to respond so hang tight.

DCF allows 30 additional days
from application date to submit
PHASE 4: APPEAL
any additional documents they
may request.

If your case is not approved after
the reconsideration phase we have

30 additional days to file an appeal
and initiate the hearing process.
PHASE 5: APPROVAL!

CONGRATULATIONS!
Your Medicaid application is approved!
PHASE 6: SMMC LTC ENROLLMENT
Remember, you still have one last step

to complete - SMMC LTC enrollment.
If you haven't done so already, it's time to
select your Medicaid HMO plan. If you're
planning to change care settings this MUST be
done BEFORE making that transition. We can

help you with this final and critical step.
