               D.1.2 Sources of Major Accident

               As to develop the sources of major accident, a qualitative hazard identification
               method is used for identification of potential sources of major accident related to
               hazardous substances and the effects.

               The processes involved are described as follows:
                   i. Hazard Identification, which includes determination of qualifying
                          substances;
                   ii. Selection of representative release cases; and
                   iii. Consequence analysis (qualitative or quantitatively).

               The sources of major accident cover the areas as follows:

                   •  Chlorine cylinder / drum storage area;
                   • Chlorine cylinder / drum handling area (loading, unloading); and
                   • Duty and standby Chlorine cylinder / drum.

               The Company Potential Sources of Major Accidents for the CIMAH reporting is
               presented in Figure D.1.2
