REquIREd INFORMATION
Today, we begin planning the celebration, Honoring Life™, while we listen and gain an
understanding of your preferences. These preferences should accomplish these three
fundamentals, to Remember, Educate & Heal™.
First . . . You Need To Know:
Our Funeral Director will call you to confirm an arrangement time and place
as well as gather initial information.
This information may include veteran discharge, healthcare power of
attorney, clergy, place of worship, cemetery, embalming consent for a
public viewing, event/service preferences Honoring Life™.
Other items may include: long sleeve clothing with a high neckline (not too
sheer), shoes or slippers, eye glasses, a recent photo for appearance and
newspaper, 25 to 50 digital photos for online media, Tribute Video and
printed materials, as well as other personal mementos to display during
the services selected.
Second . . . We Need To Know:
During our visit we gather personal history as well as other required or
needed information for the death certificate, obituary notice, printed
materials, online media, etc.
Additional information includes, but is not limited to: place of birth, social
security number, survivors, (phone number, address, e-mail), deceased
family, parents, maiden names, dates of birth, marriage, employment, job
titles, workplaces, etc.
Other information you may wish to provide includes schools attended,
memberships, hobbies, favorite pastimes, interests, and even names of
family pets.
Third . . . We Both Need To Know:
We secure payment before obituary placement and rendering of services,
as outlined in the payment policy.
This process assists the client family by eliminating the uncertainty and
financial burden.
Additionally, we provide a Statement of Funeral Goods and Services with
disclosures at the conclusion of arrangements.
