Voluntary Disability Benefits

Dewey’s Bakery offers, at no cost to you, short term (STD) and long term (LTD) disability income
benefits underwritten by Companion Life. In the event you become disabled from a non-work-
related injury or sickness, disability income benefits are provided as a source of income. A brief
overview of the benefits is provided below:

Short and Long Term Disability Income benefits are provided for disabilities that are not caused
by work-related injury or sickness. Benefits are payable to an employee during total disability
while under the regular care of a licensed physician. Total disability is defined as the employee’s
inability to engage in any occupation for which he or she is or becomes qualified by education,
training, or experience.

LTD includes a pre-existing condition limitation of 3 month look back with a 12 month wait. If
you have received treatment for a disability 3 months before the effective date, the plan will
not pay out benefits within the first 12 months after the effective date, if it is a pre-existing
condition.

No pre-existing condition limitation exists for the STD policy.
