Benefits At-A-Glance Continued





               Do you both work at Dewey’s Bakery?

               If you and your spouse are both employees of Dewey’s Bakery, there are unique rules for the

               coordination of medical, vision, dental, and life insurance coverage. In addition, there may not
               be a benefit to you to have dual coverage.  Please contact Human Resources if this situation
               pertains to you.

               When Coverage Begins


               Medical, life, voluntary life, voluntary dental, voluntary vision, short term disability, long term
               disability, and flexible spending accounts begin on the first bi-weekly pay period of the month
               following sixty days of active employment. All supplemental benefits through TransAmerica can
               only be elected during open enrollment.


               When Coverage Ends

               Dewey’s Bakery benefit coverage  ends  on the  last day of the month, in which you
               terminate employment, retire or become ineligible for benefits.

               You may be able to continue some  of your other  benefits  after  you leave Dewey’s Bakery.
               Your supplemental policies through TransAmerica (cancer, critical illness or accident) will go
               on direct bill and  you can continue paying for them  out-of-pocket at the  same  coverage
               levels and  rates if you choose.

               Some life insurance  may be continued if you contact Companion Life and  complete
               continuation paperwork within 31 days of your termination date.  You may contact
               Companion for information about the  continuation of life insurance  at 800-753-0404.
