Identification Cards

You will receive identification cards when you enroll for the benefits listed below.

If you enroll in one of the Medical/Prescription Drug
plans offered at Dewey’s Bakery, you will receive an ID
Medical/Prescription Drug card from Blue Cross Blue Shield North Carolina
(BCBSNC). You may request additional cards by
contacting BCBSNC customer service at 877-258-3334.

If you enroll in Medical, you are automatically enrolled
in this benefit. To access your plan information, please
WellQ
login to yourwellq.com. Please check the email you
provided for registration to claim your membership.

If you enroll in one of the Voluntary Dental plans
offered at Dewey’s Bakery, you will receive an ID card
Voluntary Dental from Companion Life. You may request additional
cards by contacting Companion Life customer service
at 800-753-0404.

If you enroll in the voluntary vision plan at Dewey’s
Bakery, you will receive a Member ID card from
Superior Vision. You may request additional cards by
Voluntary Vision
contacting Superior Vison customer service at 800-
507-3800. You can also visit superiorvision.com to
print out Member ID cards right away.

All FSA participants will receive a Participant ID Card
mailed to your home with a username and temporary
password that you may use to access your account
and information online at www.flores247.com. All
Flexible Spending Accounts
Medical FSA participants will receive the Benny debit
(Health Care & Dependent Care)
card mailed to your home. If you lose your card, need
additional cards or require assistance with your FSA,
please contact Flores at 704-335-8211 or our Account
