How do I enroll?
When you become eligible for the benefits at Allegacy, you will be prompted to make
your benefit elections via our online enrollment system, Ascentis. For subsequent
elections at annual enrollment periods, you will make all new benefit elections and changes in
Ascentis as well.
When Coverage Begins and Ends
Allegacy part-time new hires are eligible for benefits following a 90 day waiting period. Allegacy
coverage ends on the last day of the month following the employee’s termination for dental and vision.
Allegacy coverage ends on date of termination for all Allstate benefits. Your worksite policies through
Allstate will change to direct billing to your home and you can continue paying directly to Allstate. You
will be offered Cobra continuation for dental and vision coverage if enrolled prior to coverage
termination (see Cobra statement pages 16-18). Some life insurances may be continued and requires
you to contact the carrier and complete paperwork within 31 days of your termination date. You may
contact UNUM about continuation at 1-866-679-3054.
How/when to make changes
Unless you have a qualifying life event, you cannot make changes to the benefits you elect until the next
annual enrollment period. Qualifying life events include: marriage, divorce, legal separation, birth or
adoption of a child, change in child’s dependent status, death of spouse, death of child or other qualified
dependent, change in residence due to an employment transfer for you or your spouse, or a change in
spouse’s benefits or employment status. You have 30 days from the date of the qualifying life event to
notify Human Resources of any changes to your benefit elections.
