BERNARDON EXPERIENCE VOUCHER REQUEST FORM

Once an employee plans an experience, they should complete an Experience Voucher Request Form and submit
to the Director of Studio for the office in which the employee works. Upon approval, the monetary value of the
Experience Voucher will then be available in the form of compensation in an employee’s paycheck. Within 30
days of completion of the experience, it is required that receipt(s) associated with the completed
experience(s) are submitted to the Payroll and Benefits Coordinator.
