       To purchase Spouse &/or Child(ren) coverage, you must have coverage on yourself. Guarantee Issue is applicable at initial eligibility only.  Any   employee currently
       enrolled in Voluntary Life, not already at the Guarantee Issue amount of $100,000, will be able to increase their coverage by $10,000 without completing the EOI.  Any
       increase over $10,000, or new coverage additions, will be subject to the EOI.   *BAE Base Annual Earnings.
