SUPPLIER SERVICES AGREEMENT ADDENDUM – BUSINESS TERMS

This Supplier Services Agreement Addendum – Business Terms (this “Addendum”) to the Supplier Services
Agreement (the “SSA”), effective [month] [day], [year] and amended by the Supplier Services Agreement
Addendum, effective [month] [day], [year] and the Supplier Services Agreement Insurance/CPS Addendum,
effective [month] [day], [year] is between Centralized Supply Chain Services, LLC, a Delaware limited liability
company (the “Co-op”), on behalf of itself and its Selected Buyers, and [company], a [state] [corporation, limited
liability company, etc.] (“Supplier”). This Addendum is effective as of [month] [day], [year] (the “Effective Date”).

This Addendum amends, deletes, or supplements the Agreement as indicated and, upon execution by the Co-
Op and Supplier, is automatically incorporated into the Agreement pursuant to the terms and conditions of the
SSA. Except as otherwise modified by this Addendum, the Agreement remains in full force and effect. All
capitalized terms used, but not otherwise defined in this Addendum, have the meaning ascribed to them in the
SSA.

Section 1 - Term. (required)

Addendum or an amendment to this Addendum extending the Term (and providing for any other agreed
changes to this Addendum) as of the Termination Date, and the Agreement is still in effect, then this
Addendum continues in full force and effect until the parties either agree on the terms of a new
Addendum, or an amendment to this Addendum as described above, or the Agreement has been
terminated in accordance with its terms.

Agreement is terminated pursuant to the termination provisions of the SSA, or the parties agree on the
terms of a new addendum to supersede and replace this Addendum.

Products may be added or removed under this Addendum’s terms as mutually agreed upon by Supplier
and the Co-op and set forth in the Central Pricing System (“CPS”).
