Case Study - Meet Gloria (Age 51)
Gloria lives with her parents in a small unit on Sydney's northern beaches and was referred to the GP Social Work Program in 2021.
She has a diagnosis of Club feet and a minor intellectual disability due to childhood chemotherapy treatment.
Following a GP referral, a CCNB Social Work Care Coordinator contacted Gloria and together they identified her needs and goals. This included arranging for medical evidence and an OT report to be conducted so Gloria could get some additional support through the NDIS.
Her application was successful and now Gloria remains well and healthy in her chosen community.
