If you are ever unsure about how to perform a task safely, it is your duty to ask a qualified
person, such as the client supervisor or your Staffing Specialist for assistance. We expect all
employees to participate in our goal of accident prevention. Any potentially dangerous or
unsafe working condition must be reported immediately to the Human Resource Manager.

               SMOKING IS PROHIBITED

The use of tobacco products and E-Cigarettes is prohibited throughout the workplace and client

site. You may use tobacco and vaping products outside of the office in designated areas only.
You are not permitted to do so in front of any client office, in view of customers, or at rear
entrances unless the area is specifically designated for the use of tobacco and vaping products.
In all cases, ashes and tobacco products must be disposed of in appropriate containers.

In fairness to all employees, individuals who smoke are expected to comply with existing
policies regarding rest and meal periods.

We recognize that smoking tobacco products is legal and that employees have the right to
smoke outside of work hours. Our Company will not discriminate against any applicant or
employee based on that person’s choice to smoke.

ON THE JOB INJURIES AND INCIDENTS

You are covered under our Workers’ Compensation insurance policy. In the event of a qualified
injury or illness arising out of and in the course of employment, workers’ compensation
insurance provides for the payment of medical expenses and partial salary continuation.

Waiting periods and compensation amounts vary by state.

If you are injured or become ill on the job, you must immediately report such injury or illness to
your Staffing Specialist, regardless of whether medical treatment is required. If immediate care
is needed, proceed to the nearest treatment facility and contact your Staffing Specialist
immediately following treatment. Failure to report an injury or illness in a timely manner may
result in the appropriate workers’ compensation report not being filed in accordance with the
law, which may consequently jeopardize your right to benefits in connection with the injury or

               illness.

Employees must submit to a post-accident/incident drug screen within 24 hours of a work-
related injury or illness. Your Staffing Specialist will provide you with the form required by the
drug testing facility as well as a list of available testing sites.

Employees may refuse to consent to post-accident/incident substance abuse testing, with the
understanding that refusal to do so will result in immediate termination of employment.
