first payroll deduction.  For most benefits, insurance coverage remains in effect until the date
               of termination.


               PREMIUMS

               All employees are eligible to enroll in the voluntary Essential StaffCARE Benefits. The plan
               premiums are 100% paid by the employee, however you are able to take advantage of lower
               premiums through the group rates.  Your contributions are paid through payroll deductions on
               a post-tax basis.


               Employees who are classified as full-time, as defined by the Affordable Care Act and identified
               as full-time through the Company Look Back Period, are eligible to enroll in the Temporary
               Solutions Major Medical Health Plan.  There is an employer contribution to benefit premiums
               and the employee also pays a portion of the premium.  Premiums are deducted on a pre-tax
               basis through payroll deduction.

               PLAN MODIFICATIONS

               We explicitly reserve the right to add, modify, or eliminate, benefits, coverage, and/or
               procedures in the future, both for existing and new employees.

               BENEFITS CONTINUATION


               We offer employees group health insurance coverage (medical and dental) as a benefit of
               employment.  Under the Consolidated Omnibus Budget Reconciliation Act (COBRA), if you are
               no longer eligible for insurance coverage because of a reduction in hours, you voluntarily
               resigned, or because you are terminated for reasons other than serious misconduct, you have
               the right to continue your health insurance coverage for up to 18 months.  You will be
               responsible for paying 100% cost of this coverage plus a 2% administration fee.  If you suffer a
               disability during the first 18 months, you may be eligible for such coverage up to 36 months.


               You and your family members may have other options available to you when you lose group
               health coverage.  For example, you may be eligible to buy an individual plan through the Health
               Insurance Marketplace.  By enrolling in coverage through the Marketplace, you may qualify for
               lower costs on your monthly premiums and lower out-of-pocket costs.  Additionally, you may
               qualify for a 30-day special enrollment period for another group health plan for which you are
