Changes Coming to the 2020 AHRQ Patient Safety Survey

The Baptist Health Patient Safety Partnership (BHPSP) is preparing for the 2020 AHRQ
Survey on Patient Safety. For the upcoming year, the survey will be administered from
March 9th -30th, so please mark your calendars! The AHRQ Survey on Patient Safety is
used to raise staff awareness about patient safety, assess patient safety culture to iden-
tify strengths and areas for improvement, examine trends over time, and evaluate the
     impact of patient safety initiatives.
For the first time in over 10 years, AHRQ updated the Hospital Survey On Patient Safe-
ty (HSOPS) and is administering it as version 2.0. HSPOS 2.0 was developed by AHRQ
utilizing user and stakeholder suggestions over time. Changes include:

 Rewording complex survey items and survey items that were difficult to translate

 Adding a “Does not apply or Don’t know” (NA/DK) response option
 Shifting to a “Just Culture” framework to assess Response to Error

 Revising the staff positions and units/work areas
 Reducing the number of survey items from 51 to 40
 Reducing the number of negatively worded items

The biggest change forthcoming is the BHPSP will be administering all versions of the
AHRQ survey in both English and Spanish. In prior years, the survey was only adminis-
tered in English to our entire BHSF population; greatly limiting the participation of pri-

marily Spanish-speaking staff and/or significantly impacting data validity due to the
staff members not fully understanding the questions. The BHPSP found that it was
necessary to incorporate the Spanish surveys to be more inclusive and take into con-
sideration those departments with a higher number of primarily Spanish-speaking
staff. With the addition of the Spanish surveys, the BHPSP is expecting to see higher
participation rates and also improved percentages for BHSF overall and individual enti-
     ties.
