months. These are subm itted in three (3) copies-one w ill serve as
file of the HR in the Campus that others w ill be forwarded to the
Main Campus. The MPORs w ill be the bases o f the employees in
updating their IPCR in addition to other supporting documents
subm itted. Immediate Supervisor checks the reports and are
approved by the head of office. These reports are used in the
semi-annual assessment and preparation of OPCR quarterly
accomplishment report. Too, employees are mandated to
prepare and subm it semi-annual IPCR accomplishment report for
the period covered. These are assessed by the Immediate
Supervisor. Once approved, the reports/IPCR are assessed by the
DPIM.
b. OPCR
M onitoring sessions are conducted every m onth by the head of
the college/office. The accomplishments are reflected in the
OPCR and certified by the im m ediate supervisor. DPIM checks
and validates the subm itted OPCR. OPCRs tha t need to be revised
or lacks attachm ents are returned to the respective office while
those tha t are complete are subm itted at the COA and serve as
bases for updating the website's transparency seal o f the
institution. W ith regards to the annual assessment, Performance
Management Team (PMT) convenes to review and validate the
assessment o f actual accomplishment. The University President
makes the final assessment and approval. Once approved, the
DPIM prepares annual report and submits to the DBM, IATF, COA
and other line agencies. DPIM then updates the website's
Transparency Seal. These OPCRs will serve as bases of the DPIM
fo r the ranking o f offices. A summary report then w ill be
subm itted to the HRD Office in preparation of the Form 1 report.
c. Target Setting
Employees convene fo r the setting o f targets. The faculty/staff
then prepares their respective IPCRs based on the targets set.
The im m ediate supervisor and head o f the office reviews and
approves the IPCR targets o f the faculty/staff. Once approved,
IPCRs are subm itted at the HRD Office. Meanwhile, the head of
the office prepares his /h e r IPCR and subm it the same to his/her
im m ediate supervisor who then reviews and approved the IPCR
targets. If approved, HRD Office w ill be furnished w ith a copy for
filing.
3. As reflected by the IPCR and OPCR, one (1) faculty out of th irty seven (38)
and three (3) out o f tw enty three (23) adm inistrative staff garnered an
OUTSTANDING rem arks/perform ance fo r rating period of January to June
and July to December, 2016.
