Transportation form for busing and the textbook form. These forms need to be completed for each student
in your family. The public sending districts require the B6T transportation form in by early spring to
guarantee busing for your child. We have included extra forms in each packet for those families with more
than one student. If your family needs to make changes to your Smart account; please contact Smart directly
at 1-888-868-8828. For further assistance, please contact our tuition office at; 732-349-0018 ext. 2231.
Otherwise, your Smart tuition contract will remain from last year.

Also included for our families with students in grades K-8 re-enrolled for 2021/2022 are the
directions to apply for financial assistance. You will access the application from our website and provide all
necessary information. For further assistance with the FACTS application; please contact our tuition office at:
732-349-0018 ext. 2231. The fee to re-enroll your family for the 2021/2022 school year is $75. This non-
refundable fee will be billed to your March Smart account. If you have decided not to return for September
2021, please contact our tuition office before 2/5/2021 to avoid this fee. You will also need to complete a
transfer form for the admission’s office.
We are very excited to begin planning for the upcoming school year! Our budgeting decisions for
2021/2022 will be starting now. If your decision for your family is to remain at St. Joseph Grade School for
next school year; please complete your re-enroll packet by 2/26/2021 and receive the $50. tuition discount.
Please look for your student’s re-enroll packet and return as soon as possible. If you have any questions or
