their EZ RewardZ Card and track their transactions.

The “My Profile” Tab includes:
Personal Information. Name, Date of Birth, Business Address, Mailing Address, Phone, Email and Preferred
Communication Method are in this section. It also includes the link to “Update Profile”.

Tax Identification Information. Tax Identification Name, SSN/TIN, SSN/TIN Status and W9 Received is included in this
section. Any special messages regarding the W9 will also be in this section.

Agency Information. Agency Code, Agency Name and Linked Agencies are included in this section.

Login Information. Participant’s Login, Password Hint, Security Question and Security Answer are included in this
section.
