Long Term Disability
The group Long Term Disability Plan (LTD) is designed to provide you with income in the event you
become totally disabled and remain disabled for more than 180 days. LTD benefits will be integrated
with SDI, Worker’s Compensation and Social Security benefits. Benefits are payable through Anthem
Blue Cross Life & Health.
• Elimination Period 180 Days of Disability Benefit Percentage
• 66 2/3% Benefit Duration To Age 65 or older
• Integration Full Maximum Monthly Benefit $10,000
Self-Funded Short Term Disability
Each eligible Non-Union full time employee (that either resides in California or resides in a state that
has State Disability benefits) is entitled to our Company’s Self-Funded Short-term disability pay.
Examples of disabilities or situation that might warrant use of this plan are (but not limited to):
• Pregnancy disability leave or family medical leave because of his or her own serious health
condition.
• Each eligible employee may participate once in any twelve-month period for no more than 22
days.
• Eligibility
A Non-Union full time employee is eligible short-term disability pay if he or she:
• has worked for our Company for at least six months and
• regularly works at least thirty hours per week and
• has exhausted all available paid sick leave and
• has not participated in this program in the prior twelve months and
• is on pregnancy disability leave or on family leave because of his or her own serious health
condition, and
• is receiving State Disability Insurance.
Amount of Pay
Company Funded Short Term disability pay is the difference between the eligible employee’s regular
straight-time weekly rate of pay when the leave begins and the weekly amount the employee receives
from State Disability Insurance from the 8th through the 30th working day of the working day period.
Procedure and Notice
All employees must notify Human Resources immediately once they are aware of a possible need for
Short and/or Long Term Disability assistance. Employees are responsible for contacting and
communicating with their State Disability Insurance program.
