Long Term Care
LTC Employer Paid Base Plan – all full time non-union employees are automatically enrolled into this
base plan paid by Sharks Sports & Entertainment.

Automatic Coverage
• Monthly Benefit: $2,000 (you can also purchase up to $9,000*)
• Benefit Duration: 3 Years (you can also purchase 6 years) or (unlimited duration*)
• $72,000 Lifetime Maximum
• Completely Portable
Optional Coverage Paid by the Employee
• Inflation Protection — Base Plan does not include Inflation Protection but is available as an
optional buy up (Simple and Compound Options)
• Coverage available to Family Members Including: Parents (In-Law), Spouse (DP), Adult
Children, Siblings (all family members coverage requires completion of medical questionnaire)
Reason to Purchase Optional Long Term Care Insurance During
Initial Enrollment:
• Price: Your costs through this offering are typically significantly less expensive than purchasing
an individual plan on your own.
• Guarantee Issue: As an employee, most of the coverage is provided to you as guarantee issue,
meaning regardless of your medical condition, you qualify. Guarantee issue is only available
during the initial enrollment.
• Lock Yourself Into an Age: Rates do not go up as you age. The younger you buy, the less
expensive your costs will be because you lock yourself into that age.
• Freedom and Flexibility: The program pays a benefit to you and you can decide how to spend
it. This indemnity based model pays your benefit regardless of cost of care or U.S. location
where you receive the care.
• Portability: If you ever leave, retire or reduce your hours, you can take this coverage with you.
Unum will bill you on a direct basis.
