Core Indicators

 Multiple hospitalizations and/or ER  Increasing pain requiring increasing
visits analgesic dosing
 Unintentional weight loss (>10%) in  Advanced disease ‐ unstable,
past six months deteriorating, complex symptom
burden
 Limited self‐care, in bed or chair ≥
50% of day  Decreasing response to treatments,
decreasing reversibility
 Increasing dependence in ADLs
 Choice of no further active treatment
 Decline in functional performance
status  Progression of Stage III‐IV pressure
ulcers despite optimal care
 Serum albumin <2.5g/dl
