Why Advance Care Planning?
• Introduction of Advance Care Planning is important
for all patients upon admission to the CHHA
• Identification at point of Intake of patients who need
an in-depth advance care planning conversation, or
who have palliative or hospice needs
• Enhanced clinical assessments by CHHA clinicians of
advance disease if appropriate, to better identify
patients with palliative or hospice needs
Palliative and Hospice Care-
What’s the difference?
Palliative care is focused on providing
patients with relief from the symptoms,
pain, and stress of a serious illness ‐
whatever the diagnosis
Palliative care is appropriate at any age and
at any stage in a serious illness, and can be
provided together with curative treatment
Hospice Care provides compassionate,
comfort‐oriented end of life care aimed at
improving quality of life
Hospice care settings can include the
patient’s own home, a nursing
home/residence, or a specialized in‐patient
unit
