Special Considerations
When in doubt Confirm with your CRM/CFM

• Fluoroscopy PICC insertions
- VNSNY Patient Facing Staff will need to retrieve the insertion report for
fluoroscopy
• Tips in the Right Atrium
- Infusion Admin has to approve these types of cases before Intake
releases them
• If the infusion Transfer to Inpatient Facility (TIF) case has been
hospitalized for over 48 hours
- Intake must obtain updated infusion orders
• Free Care:
- Free care Infusion referrals have to be approved by Infusion Admin, the
vendor will absorb all the costs of medication and supplies
• Pediatric Infusion Referrals:
- The VNSNY Infusion program follows the CHHA alignment for PED cases.
If CHHA does not provide PED services in a specific region, VNSNY
cannot provide infusion services either

Same Day Visits

• Intake must contact the CFM for same day visits
- Schedulers and CFM ensure afternoon visit is possible
- Intake must document their conversation in Coordination note

• RSOC is the date the Vendor starts
