Special Considerations
• Long term cases are those longer than 8 weeks, the preference is to
have another CHHA take on these cases. This would need approval by
a Clinical Field Manager and Clinical review manager . Patients that
are unable to administer their infusion therapy or have no caregiver
support are not usually accepted. Branch Director/ CFM/CRM and
infusion operations must be consulted for this type of issue.
• Referrals that are considered low utilization (< 5 days) cases are
to be discussed with a CFM/CRM

Patients on VNSNY CHHA services with Managed Care
Insurance going home with infusion services
• Even if VNSNY does not accept managed care insurances for
infusion – patients requiring skilled services would still be
accepted and the vendor will provide the infusion care
• The VNSNY Intake will document in Coordination notes the
name and number of the company providing Infusion for the
patient
• The VNSNY Intake staff has to list all medications (CHHA meds
and Infusion meds) *** The IC only has to put in one med***
