                                  Required Documentation at Intake
                                  Labs
                                   Labs: all below require trough levels and bun/creatine
                                     levels (must be within 7 days) Depending on the hospital
                                     site or referring facility
                                      - Vancomycin (10‐20 mg/L)
                                      - Gentamycin (0‐3 mg/L)
                                                                   If any of these troughs levels fall outside the
                                      - Amikacin (<10 mg/L)        range contact your CFM/CRM
                                      - Tobramycin (<2 mg/L)


                                   Lab levels for:
                                       chemistry ( CMP) and CBC should be obtained if available
                                       Trough levels w/BUN/Creatinine  if applicable should be
                                         documented

                                 • All pertinent documentation must be attached to Worldview
                                    and attached to HCHB


                                      Signed drug Scripts and lab orders IV Access Device type: (PICC,
                                        Port, Tunneled, PIV, Midline)
                                      Insertion Report/Chest X‐Ray/ECG (with acceptable tip location)
                                      If a patient requires PT/INR to be drawn more than 1x/week, the
                                        acute team must place the order (Intake staff must inform the
                                        Operations team). When patients are on Coumadin they usually
                                        require PT/INR draws


                                  *Labs related to Infusion will be managed by Infusion Vendor. Non
                                 infusion related labs, will be ordered by Clinical Operations
