What is Needed to Process an Infusion Referral?
• Most IV Access Devices must be centrally located (SVC or
SVC/RA, Cavoatrial junction)
• If the device is not centrally located:
- confirm with physician if the line can be repositioned
• If the PICC line is located in the right atrium it is acceptable
with adequate documentation (MD scripts) Please contact
Infusion before accepting any cases with Tips in this
placement.
• If PICC, PORT, Tunneled not centrally located, there must be
supporting documentation:
- Progress note or script by MD
- MD approval must be indicated on progress note or script to use
current location. Please contact Infusion
• Vancomycin should be administered centrally, however VNSNY
can accept Vancomycin via the midline catheter for 5 days as
long as the Vendor administers the medication
