IA’s Process within Intake

 Check your assignments from supervisor
 Go to Worldview. If referral is new, select “MR Referrals”. If you are
uploading an additional attachment to an existing record, select “MR
Episode” from the drop-down menu to find out status of the case
(known, active or discharged).
 DOB & gender is all the information needed for Worldview.
Information then gets transferred to Homecare Homebase from
Worldview.

 Enter the patient’s name, date of birth, gender, and address to
create a case shell in Homecare Homebase.

 Fill in the screens until the “Demographics” tab. Use “Clinical” tab to
enter facility information (name of facility referring patient).

 After this, the case goes to the assigned IC.
