                             Intake Is The Starting Point For Homecare


                             Intake acts as the liaison between the referrer or customer
                               and VNSNY to initially receive a homecare referral
                                • Relationships are built by Intake staff and the customer,
                                  which ensures continued referral business
                             Intake will evaluate the patient for appropriateness for
                               homecare
                             Intake staff will work with to gather data elements needed to
                               complete minimum data set, seek patient acceptance of
                               homecare, and ensure appropriate physician orders are
                               received upon onset of referral

                             Upon completion of Intake data set, Intake will pass referral
                               to Operations
