Initial Patient Assessment: General Tips

 Ask patient if they have ever had home care
before
- if no, describe home care services;
- if yes, ask them to describe their experience

 Ask clearly and concisely if patient would like you
to assess them for homecare services
- Do not make any promises, set expectations
accordingly (limitations of insurance, daily visits)

Verifying Visit Address

 Always verify the address including apartment #
and the telephone #
 Ask the patient or family member to give you the address and phone
number
 Don’t just read it to them from face sheet expecting them to agree
that the info is correct
 Verify emergency contact name and telephone#

 How can the nurse enter the building?

 Is the bell or buzzer working?
 Is the entrance on the street listed on the address or on a side street?
 Is there a code for patients who live in gated communities?
 Does the patient own pets?
