                   2.  Intake staff or CFM will notify the referrer and/or the physician and will document
                       in the coordination note reason for non-admission and referrer/ physician
                       notification.

                   3.  Branch Director reviews all Non-Admit cases and can reverse Non Admit if
                       appropriate.

                   Not Admit of Accepted Referral

                   1.  Circumstances may prevent VNSNY Home Care from initiating service after a
                       referral has been reviewed and approved and start of episode date planned.
                       These may include but are not limited to patient/family refusal of care at home or
                       when the patient returns to hospital or other facility before planned start of
                       episode or any unsafe situation identified in the home.
                   2.  A patient who does not meet admission criteria or who is assessed as
                       inappropriate for VNSNY Home Care services by the nurse or therapist on the
                       initial home visit will not be deemed as an admission to VNSNY Home Care. The
                       patient's physician and referring party and the clinical field manager will be
                       notified by the nurse or the therapist who made the initial assessment.
                          a.  During the initial home visit, the nurse or the therapist must address the
                              patient's minimal health and safety needs and develop an alternate, more
                              appropriate plan for community services to meet these needs.
                          b.  Assisting a patient with obtaining alternate services may include
                              transferring the patient to the hospital if the patient is found to be unsafe at
                              home. In such cases, the nurse and/or other designated person, remains
                              with the patient until he/she is removed from the home.
                          c.  The nurse then contacts the referring physician to discuss the patient's
                              status, reasons for transfer to the hospital and reasons why the patient
                              cannot be managed safely at home.

                   3.  CFM will update referral as Non–Admit and will document reason and will notify
                       referrer of same.

                   4.  At the time when determination is made to deny a patient admission, VNSNY
                       staff shall also determine whether the patient appears to be eligible for services
