1. Skilled Clinician (RN or therapist) determines patient care needs and level
of assistances needed with personal care (ADLs, IADLs) and obtains
physician orders for the HHA services.
2. The first HHA visit is to be provided as requested on the plan of care
contingent upon entry of authorization.
3. If authorization for the HHA services is denied, Patient / family are to be
informed that services may be provided if they wish to pay privately or be
considered for charity care
