About Provider Services
VNSNY’s Provider Services is comprised of:
• Home Care
• Community Mental Health Services
• VNSNY Hospice and Palliative Care
• Partners in Care
Care from Provider Services is short-term and provided by
an interdisciplinary care team that may be comprised
of home health care services such as skilled nursing,
rehabilitation therapy, social work, behavioral health
and personal care services by home health aides.
