Skilled Nursing
VNSNY provides
skilled nursing care
that incorporates best
practices based on
established national
guidelines, including
the American Heart
Association and
the Visiting Nurse
Association of America. Our goal is to maximize patients’
physical, cognitive, social, and behavioral well-being to
achieve a higher level of independence at home and in the
community. Our care and services help reduce avoidable
rehospitalizations and hospital length of stay, improve
recoveries, and assist with safe transitions across settings.
Services include:
• Assistance with adherence to doctors’ orders
• Pain management and medication reconciliation
• Instruction in self-management of disease
• Assistance with symptoms management
• Specialized care for patients with high risk of
hospitalization, including care for post-surgical
wounds and other complex treatment
Service Area: All ve boroughs of New York City, Nassau,
Su olk and Westchester Counties
