 Measure the stoma on each visit as
stoma will shrink during first 6
weeks. Use a measuring guide – if
not available measure in inches
 Have pt sit up and stand to
evaluate for skin crevices, folds and
depressions making pouching a
challenge
 Pull skin taut above and below
stoma to make “oval” shape round
