Peristomal Assessment

 Complications: Deep Peristomal creases causing
poor fit of appliance can have erythemic painful
denuded skin as a result
 Assessment :
Look for abdominal folds, creases have patient
sit, stand and lie down to assess

 Management
Usually need all-flexible wafer that can mold to
the patient’s abdominal contours
Creases at 3 and 9 o’clock - use strips to fill in the
creases and create an even pouching surface
Dust peristomal skin with stomahesive powder
and then crust with skin barrier or water, fill in
deficits with strip paste or barrier ring

 Cleanse the separation and fill
with stomahesive powder and
