           reimbursement:
               Able to identify primary and secondary
               payers, and can
               Verbalize whether it is a managed care,
               straight Medicare or Medicaid.
        2.  For managed care cases:
               Can identify if the patient has authorization.
               Demonstrates ability to escalate for timely
               visit authorization.
        3.  Demonstrates ability to complete subsequent visit
           note documentation with minimal corrections.
        4.  Demonstrates ability to complete OASIS
           documentation accurately.
        5.  Demonstrates ability to establish an appropriate
           POC to support physical, social and psychological
           needs, and address environmental conditions.
        6.  Able to sign off all notes independently.
        7.  Demonstrates ability to sign off OASIS
           documentation within 24 hours
        8.  Demonstrates ability to sign off 80% of
           subsequent visit notes on the same day.
        9.  On admissions and subsequent visits, assures
           continuity of safe and effective care for patients in
           collaboration with CFM /CRM.
        10. Demonstrates skills related to care management
        of caseload; care coordination and interdisciplinary
        collaboration.
        11. Demonstrates customer service behaviors and
        communication skills that achieves the patient
        perception of service excellence.
