        1.  Documents within established guidelines: assessment data and
            findings, nursing interventions and responses to interventions and
            outcome.
        2.  Demonstrates ability to set priorities based on acuity of patient
            need.  Gives rationale for actions or decisions based on one or
            more of the following:
               Scientific principles/logic
               Successful experience
               Clinical reasoning
               Standards of practice/protocols (Center of Excellence)
        3.  Develops or revises a plan of care for assigned patients based
            upon patient progress, change in status, patient and caregiver
            preferences.
        4.  Identifies and escalates complex case-related issues timely and
            appropriately to the CFM (i.e. utilize complex case trigger form).
        5.  Demonstrates skills related to care management of caseload, care
            coordination and interdisciplinary collaboration.
        6.  Demonstrates interpersonal communication skills with patients
            and other involved disciplines that is focused on achieving
            desirable outcomes and responses.
        7.  On admissions and subsequent visits, assures continuity of safe
            and effective care for patients in collaboration with CFM /CRM.
        8.  Demonstrates knowledge and skills to manage, plan, coordinate
            and execute the patient’s safe discharge from service.
        9.  Verbalizes knowledge related to managing LUPAs and visit
            utilization to achieve desired quality and financial outcomes for
            each patient.
        10.  Demonstrates customer service behaviors and communication
            skills that achieves the patient perception of service excellence.
        11.  Able to consistently sign off OASIS documentation within 24 hours,
            and 80% of subsequent visit notes on the same day.
