 Demonstrate ability to escalate for timely
visit authorization.
4. Demonstrates ability to complete

subsequent visit notes with minimal
corrections.
5. Demonstrates ability to sign off visit notes
independently.
6. Demonstrates ability to sign off 80% of
subsequent visit notes on the same day.
7. Based on outcome of visit, communicates
pertinent findings and clinical changes as
appropriate to Care Manager and CFM.
8. Escalates complex case-related issues timely
and appropriately to the Care Manager and
the CFM (i.e. utilize complex case trigger
form).
9. Demonstrates customer service behaviors
and communication skills that achieves the
patient perception of service excellence.
10. Compliant with established personnel
policies /organizational mandates.
