specific information regarding each Clinical Pathway, please refer to the Clinical Pathways link located on
the VNSNY Intranet Homepage. Remember, the Clinical Pathways serve as a guide to treatment. If
clinical judgement dictates that the patient needs more (or less) than the recommended number of visits,
then make sure to justify so in your documentation, and inform your manager.
Enter the physician you are sending thse orders to, as well as any other MDs involved in the case (Copy to
Physician). Enter order date and time. If Verbal Orders are obtained, enter the date and time you obtained
the VO. If unable to obtain VO, UNCHECK VERBAL ORDER.
Click on View Instructions to include your selected Pathways in the order. Click on Add Calendar to map
out your visit frequency and duration.
