 To claim an exclusion for accelerated
death benefits made on a per diem
or other periodic basis, you must file
Form 8853, Archer MSAs and Long-
Term Care Insurance Contracts, with FORM 8853
your return.
 You don’t have to file Form 8853 to
exclude accelerated death benefits
paid on the basis of actual expenses 68
incurred.
